Primary Care

Primary care services are for maintaining good health with regular health check-ups. Primary care includes health counseling and education, diagnosis, and treatment of acute health conditions, support for managing routine health screenings and exams, and long-term healthcare needs, including chronic health conditions.

Diabetic Care

Our diabetes care plans help diabetics manage diabetes in their day-to-day lives. These plans are helpful, as diabetes is a long-term condition that often requires a person to change their diet, lifestyle, and daily routines.

Asthma/Allergies

We’ll help you keep your asthma and/or allergies under control with a care plan.

Women's Wellness

Services are for maintaining good health with regular check-ups for women. Women’s Wellness includes adolescent gynecology, breast exams, pelvic exam/PAP smears, and evaluation of past PAP results.

Pediatric Care

Children and adolescents are treated by our pediatricians and Family Nurse
Practitioners who provide care, sick visits, school physicals, and medication
management.

Treatment of Chronic Medical Conditions

Our Family Nurse Practitioners provide quality medical care for adults with
chronic conditions such as diabetes, hypertension, high cholesterol, asthma/allergies, arthritis, and thyroid health issues.

Prescription Assistance

Your medication to maintain good health is important. We will help you get free or low-cost brand-name medications.

Counseling

Our licensed counselor provides emotional support and assists with identifying and practicing healthy ways of dealing with some of life’s biggest challenges. Services are free to our patients.

Telehealth

Certain Chronic Health care needs and the overall patient’s well-being may be addressed
virtually.

Step 1: Do You Qualify?

Resident of Collin County

CHC serves the uninsured residents of Collin County.

Uninsured

CHC provides non-urgent, primary healthcare to the uninsured. Patient may not be covered by other health insurance or medical assistance program.

Within Income Guidelines

Household income of less than: 200% of the Federal Poverty Level Guidelines.

Step 2: First-Time Patients

Become a Patient

Please call (972) 547-0606 to schedule an enrollment appointment to verify the qualifications.

Prospective patients must bring the following documents listed below.

Future medical appointments will only be provided after receiving all required documentation.

Enrollment appointments for new patients every Wednesday from 8:30 AM to 1:00 PM.

Step 3: Prepare Your Documentation

You will need to bring ALL the following documents to Admissions.

Proof of Address

Please provide the following documents:

  • Driver’s license or valid issued photo ID
  • Current residential utility bill with patient name or rental/lease agreement with patient name and current address
Proof of Income

Please provide one or more of the following documents:

  • Two recent pay stubs
  • Income tax return from the most current year
  • Proof of child support
  • Social Security benefits
  • Unemployment benefits
  • Disability benefits
  • Worker’s compensation
  • Welfare assistance
  • Commissions, fees, and tips
  • Government-funded housing
  • Food Stamps
  • Letter from employer

What You Need to Know

Think about what you would like to discuss with your doctor. Is there anything concerning you would like to discuss with your doctor? Have you thought about getting a flu shot? Are you concerned about how your condition will affect your daily life? Organize your thoughts and ask about the most important matters first if you have more than a few items to discuss. Don't wait until the end of your appointment to bring up the things that are really bothering you -- bring them up right away.
